How do we know a patient balance is actually ready to be sent to collections?<\/span><\/h4>\n<p data-start=\"1318\" data-end=\"1630\">Before placement, confirm that insurance payments and contractual adjustments are complete, patient payments and credits have been posted, and legitimate billing questions have been resolved. The amount sent to collections should represent the patient&#8217;s <strong data-start=\"1572\" data-end=\"1597\">actual responsibility<\/strong>, not simply the original charge.<\/p>\n<p data-start=\"1632\" data-end=\"1831\">The CFPB specifically recommends reviewing insurance payments, duplicate charges and whether the patient actually owes the amount before medical debt is pursued.<\/p>\n<h4 data-section-id=\"1pxeey6\" data-start=\"1833\" data-end=\"1927\"><span style=\"color: #800000;\">2. Should we send an account to collections while an insurance appeal is still unresolved?<\/span><\/h4>\n<p data-start=\"1929\" data-end=\"1968\">Usually, resolve the uncertainty first.<\/p>\n<p data-start=\"1970\" data-end=\"2171\">If an insurer may still pay another $600, don&#8217;t send the patient a $600 collection demand and correct it later. Determine the final patient responsibility whenever reasonably possible before placement.<\/p>\n<p data-start=\"2173\" data-end=\"2277\"><strong data-start=\"2173\" data-end=\"2239\">Collections should begin with a reliable balance, not a guess.<\/strong><\/p>\n<h4 data-section-id=\"6ezoev\" data-start=\"2279\" data-end=\"2353\"><span style=\"color: #800000;\">3. What documentation should match the amount we place for collection?<\/span><\/h4>\n<p data-start=\"2355\" data-end=\"2596\">At minimum, your account ledger should reconcile with the amount assigned. Depending on the account, supporting records may include statements, dates of service, payments, insurance adjustments, credits and the responsible-party information.<\/p>\n<p data-start=\"2598\" data-end=\"2717\">If your system says <strong data-start=\"2618\" data-end=\"2633\">$1,275 owed<\/strong> but your documentation supports only <strong data-start=\"2671\" data-end=\"2681\">$1,050<\/strong>, resolve the $225 difference first.<\/p>\n<p data-start=\"2719\" data-end=\"2786\">A strong collection file is easier to explain, validate and defend.<\/p>\n<h4 data-section-id=\"1owb5ls\" data-start=\"2788\" data-end=\"2871\"><span style=\"color: #800000;\">4. How much patient information should we give a collection agency under HIPAA?<\/span><\/h4>\n<p data-start=\"2873\" data-end=\"2934\">Only what is reasonably necessary for the collection purpose.<\/p>\n<p data-start=\"2936\" data-end=\"3273\">HIPAA specifically permits healthcare providers to use collection agencies as part of their payment activities, generally through an appropriate business-associate relationship. However, HIPAA&#8217;s <strong data-start=\"3131\" data-end=\"3162\">minimum-necessary principle<\/strong> still applies to protected health information used for payment purposes.<\/p>\n<p data-start=\"3275\" data-end=\"3379\">A collector normally does not need the patient&#8217;s entire medical chart just to recover an unpaid balance.<\/p>\n<h4 data-section-id=\"5ymiwc\" data-start=\"3381\" data-end=\"3464\"><span style=\"color: #800000;\">5. What should a medical collection agency do when a patient disputes the bill?<\/span><\/h4>\n<p data-start=\"3466\" data-end=\"3529\">It should investigate rather than simply increase the pressure.<\/p>\n<p data-start=\"3531\" data-end=\"3794\">The practice may need to verify insurance adjustments, payments, charges or responsible-party information. Regulation F also contains specific validation and dispute procedures for applicable third-party consumer collections.<\/p>\n<p data-start=\"3796\" data-end=\"3909\">A good collection agency should have a clear route for sending legitimate billing questions back to the provider.<\/p>\n<h4 data-section-id=\"1516b6u\" data-start=\"3911\" data-end=\"3999\"><span style=\"color: #800000;\">6. Can a collection agency discuss a medical bill with a spouse, parent or guardian?<\/span><\/h4>\n<p data-start=\"4001\" data-end=\"4030\">Sometimes, yes\u2014but carefully.<\/p>\n<p data-start=\"4032\" data-end=\"4428\">HHS says HIPAA permits providers and their collection agencies to communicate with other parties when necessary to obtain payment, while still limiting protected health information to what is reasonably necessary and honoring applicable confidentiality restrictions. Regulation F also contains specific rules involving spouses and parents of minor consumers.<\/p>\n<p data-start=\"4430\" data-end=\"4548\">The important distinction is <strong data-start=\"4459\" data-end=\"4547\">discussing payment appropriately versus unnecessarily disclosing medical information<\/strong>.<\/p>\n<h4 data-section-id=\"l77fb1\" data-start=\"4550\" data-end=\"4637\"><span style=\"color: #800000;\">7. What should nonprofit hospitals check before using stronger collection measures?<\/span><\/h4>\n<p data-start=\"4639\" data-end=\"4660\">Financial assistance.<\/p>\n<p data-start=\"4662\" data-end=\"4944\">Tax-exempt hospitals subject to IRC Section 501(r) must maintain financial-assistance policies and make reasonable efforts to determine whether an individual qualifies for assistance before taking specified <strong data-start=\"4869\" data-end=\"4905\">extraordinary collection actions<\/strong>.<\/p>\n<p data-start=\"4946\" data-end=\"5169\">That makes coordination between the hospital&#8217;s billing department and collection agency critical. An account should not simply move from \u201cpast due\u201d to aggressive recovery without checking the hospital&#8217;s applicable policies.<\/p>\n<h4 data-section-id=\"17tg67c\" data-start=\"5171\" data-end=\"5247\"><span style=\"color: #800000;\">8. Is a high recovery rate enough to choose a medical collection agency?<\/span><\/h4>\n<p data-start=\"5249\" data-end=\"5252\">No.<\/p>\n<p data-start=\"5254\" data-end=\"5483\">A quoted recovery percentage can be almost meaningless unless you know <strong data-start=\"5325\" data-end=\"5354\">what accounts produced it<\/strong>. A portfolio of fresh $500 balances should not be compared with five-year-old accounts that previous agencies already attempted.<\/p>\n<p data-start=\"5485\" data-end=\"5509\">Ask for metrics such as:<\/p>\n<ul data-start=\"5511\" data-end=\"5683\">\n<li data-section-id=\"10pltzd\" data-start=\"5511\" data-end=\"5537\">recovery by account age;<\/li>\n<li data-section-id=\"chlh4i\" data-start=\"5538\" data-end=\"5566\">recovery by balance range;<\/li>\n<li data-section-id=\"1nempzy\" data-start=\"5567\" data-end=\"5584\">complaint rate;<\/li>\n<li data-section-id=\"8g2y4b\" data-start=\"5585\" data-end=\"5600\">dispute rate;<\/li>\n<li data-section-id=\"6ea7i5\" data-start=\"5601\" data-end=\"5625\">time to first payment;<\/li>\n<li data-section-id=\"1a445pv\" data-start=\"5626\" data-end=\"5652\">payment-plan completion;<\/li>\n<li data-section-id=\"91oqbk\" data-start=\"5653\" data-end=\"5683\">and net recovery after fees.<\/li>\n<\/ul>\n<p data-start=\"5685\" data-end=\"5781\"><strong data-start=\"5685\" data-end=\"5781\">Recovery rate tells you how much came back. It doesn&#8217;t tell you what happened along the way.<\/strong><\/p>\n<h4 data-section-id=\"t8n46\" data-start=\"5783\" data-end=\"5878\"><span style=\"color: #800000;\">9. Why should we read patient reviews of a collection agency\u2014not just reviews from doctors?<\/span><\/h4>\n<p data-start=\"5880\" data-end=\"6017\">Because doctors tell you what it is like to <strong data-start=\"5924\" data-end=\"5932\">hire<\/strong> the agency. Patients tell you what it may be like to <strong data-start=\"5986\" data-end=\"6005\">be contacted by<\/strong> the agency.<\/p>\n<p data-start=\"6019\" data-end=\"6145\">Look for patterns involving courtesy, pressure, dispute handling, payment arrangements, responsiveness and incorrect balances.<\/p>\n<p data-start=\"6147\" data-end=\"6306\">For healthcare providers, this is especially important because the patient may associate the collector&#8217;s behavior with <strong data-start=\"6266\" data-end=\"6305\">your practice&#8217;s name and reputation<\/strong>.<\/p>\n<h4 data-section-id=\"1ychyzt\" data-start=\"6308\" data-end=\"6428\"><span style=\"color: #800000;\">10. What happens if the patient pays our medical practice directly after we already sent the account to collections?<\/span><\/h4>\n<p data-start=\"6430\" data-end=\"6493\">The agency and provider need a reliable reconciliation process.<\/p>\n<p data-start=\"6495\" data-end=\"6644\">If the patient pays your office Monday but the collector calls Wednesday demanding the same balance, you have created an avoidable patient complaint.<\/p>\n<p data-start=\"6646\" data-end=\"6850\">Your agency should have a straightforward process for reporting <strong data-start=\"6710\" data-end=\"6777\">direct payments, adjustments, cancellations and balance changes<\/strong> so the collection system stays synchronized with the provider&#8217;s records.<\/p>\n<h4 data-section-id=\"d8v9jl\" data-start=\"6852\" data-end=\"6914\"><span style=\"color: #800000;\">11. How important are a client portal and EHR integration?<\/span><\/h4>\n<p data-start=\"6916\" data-end=\"6959\">More important as account volume increases.<\/p>\n<p data-start=\"6961\" data-end=\"7103\">For five placements per month, manual entry may be manageable. For hundreds or thousands, duplicate data entry becomes an operational problem.<\/p>\n<p data-start=\"7105\" data-end=\"7343\">A good platform should support bulk uploads, account-status visibility, payment reporting and secure information exchange. Integration with systems such as <strong data-start=\"7261\" data-end=\"7274\">athenaOne<\/strong> can further reduce manual account transfers and reconciliation work.<\/p>\n<p data-start=\"7345\" data-end=\"7458\">The technology should remove work from the billing department\u2014not create another system the staff has to babysit.<\/p>\n<h4 data-section-id=\"15z9xuz\" data-start=\"7460\" data-end=\"7542\"><span style=\"color: #800000;\">12. Should newer and older medical accounts use the same collection fee model?<\/span><\/h4>\n<p data-start=\"7544\" data-end=\"7560\">Not necessarily.<\/p>\n<p data-start=\"7562\" data-end=\"7712\">Newer balances may justify a low-cost <strong data-start=\"7600\" data-end=\"7622\">fixed-fee approach<\/strong>, while older, harder-to-recover accounts may be better suited to contingency collections.<\/p>\n<p data-start=\"7714\" data-end=\"7972\">CA-USA, for example, offers a <strong data-start=\"7744\" data-end=\"7768\">$15 fixed-fee option<\/strong>, where the debtor pays the provider directly and the provider keeps 100% of the recovery.
